Membership: The 1st five forms are for parents to use for sign-up and the last two are to be used for positive and negative feedback regarding your child's coach(s). The MHSAA Physical form has two sides and can be typed direct into from your computer. You can then print and take the form to your doctor to be completed.
Coaches: The first three forms are required by GLAYF and Tri-County Youth Football. If you desire to be a coach you will need to complete all 3 of these. The final form is used to evaluate officials after your games. Complete this form as soon as possible after a game. These PRE-Season forms must be turned in by the deadline of May 17th.
